    The Milwaukee School of Engineering's (MSOE) Master of Science in Perfusion (MSP) Program currently uses paper documents to record case participation and to evaluate students during their clinical rotations. When a student participates in a clinical procedure they are tasked with documenting their participation with a case log form while the instructor is tasked with completing a separate evaluation of the student. This method of documentation and evaluation has been a useful tool for MSOE MSP students and staff. But there are certain challenges associated with maintaining the paper forms, mainly the efforts involved in distributing, collecting, and collating the data. This thesis project describes the development of a system whereby the student case log and evaluation forms can be completed electronically and the data automatically collated on a per student basis.
    Several methods were investigated for incorporating the case log forms into an electronic format with databasing capabilities. The initial focus was to create a private website that would be completely customizable. This method was abandoned due to the associated costs. Several other options were also considered, including Blackboard, fillable Adobe PDF forms, fillable Adobe LiveCycle, and Google Forms. Each of these four options were developed and tested in order to evaluate their respective strengths and weaknesses.
    The final platform implementation includes the use of Adobe PDF forms for the case log forms and Google Forms for the recording of evaluations. This combination of platforms was chosen to maximize their respective benefits. The Adobe PDF form was chosen for the case log form primarily because of its compatibility with all tested operating systems and its familiarity with the end-users. Google Forms was selected for its ease of use and its ability to automatically collate submitted data. This combination of platforms will provide a much more efficient process of distributing, collecting, and collating the student clinical data.
